SDI Claim Issue: Can't Get Continued Claim Certification Form After Doctor Updated Return-to-Work Date
I'm having trouble with my SDI claim. My doctor updated my return to work date and submitted a supplemental certification to continue my benefits, but now my account shows I need to submit a Continued Claim Certification form. The problem is, I haven't received this form in my inbox or mail. Has anyone else experienced this? What should I do to get this form?
