EDD Disability Claim: Need help identifying doctor's form after 30 days without payment

Hey everyone, I'm having trouble with my disability claim. I haven't been paid in 30 days and I'm trying to figure out what form I need from my doctor. I talked to someone last week but I'm still confused about the exact name of the form. Anyone know what it might be called or have experience with this?

I found a way to actually get someone on the phone! I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/1X-mEsLtbmQ
