EDD Disability Claim: Need help identifying doctor's form after 30 days without payment
Hey everyone, I'm having trouble with my disability claim. I haven't been paid in 30 days and I'm trying to figure out what form I need from my doctor. I talked to someone last week but I'm still confused about the exact name of the form. Anyone know what it might be called or have experience with this?

Sounds like you need the Physician/Practitioner's Certificate form. It's pretty common for disability claims. Have you checked the EDD website? They usually have all the forms listed there.

I found a way to actually get someone on the phone! I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/1X-mEsLtbmQ

Pro tip: If you can, go to an EDD office in person. Sometimes you can get things sorted out faster that way. Bring all your paperwork with you!
