Do I need special EDD forms for disability extension when benefits end this month?

Hi everyone, I'm getting a bit anxious because my disability benefits are scheduled to end at the end of this month, but my doctor says I'm not ready to return to work yet. I've been on disability since February after my back surgery, but recovery is taking longer than expected. Do I need to get my doctor to fill out a specific form for an extension? Is it the same form as the original claim? I've heard different things from friends and I'm confused about the process. The EDD website mentions something about 'continued claims' but I can't figure out exactly what I need to do. Anyone gone through this recently? I really can't afford to have a gap in payments.

Yeah, ur doc needs to do the continuation form. It's like the first one but different. I got mine extended twice last yr for my knee.

This is NOT accurate!!! EDD is a NIGHTMARE to deal with for extensions. My doctor submitted my extension form THREE TIMES and EDD kept saying they never got it. I ended up going 6 weeks with NO payments because of their incompetence. ALWAYS get proof of submission from your doctor and follow up with EDD directly!!!

But here's the important part - if you don't see the extension reflected in your SDI Online account within about 5 days after your doctor submits it, CALL EDD immediately! I waited 2 weeks assuming everything was fine, and then suddenly my payments stopped. It was a nightmare trying to get through to someone. Oh also, make sure your doctor puts the right dates on the form. The start date for the extension should be the day after your current disability period ends. My doctor's office messed this up the first time and it caused delays.

Let me just add something here that people might not realize - if your doctor extends your disability, EDD might FLAG your claim for review!!! This happened to me and I had to do a PHONE INTERVIEW with an EDD representative to explain why I needed more time. They asked me all kinds of personal questions about my condition and treatment plan. It was very stressful!!! Just be prepared for this possibility, especially if this is your second or third extension. They're really cracking down on longer claims in 2025.

btw once ur doc does the form u should see it update in ur online account like a few days later. if u dont see anything changing after a week somethings wrong

I wanted to update everyone - I talked to my doctor yesterday and he was familiar with the Supplemental Certificate form. He submitted it electronically right after my appointment!
