SDI Form Delay: Last Payment in December, Need Form for Doctor ASAP - Help!

Hey everyone, I'm in a bit of a pickle with my disability payments. My last payment was in December and I'm still waiting for the form to continue my disability. I need to take it to my doctor soon, but I haven't received anything yet. Has anyone else experienced delays like this? Any advice on how to speed up the process?

I found a way to actually get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/1X-mEsLtbmQ
