I'm having trouble with my EDD claim. My doctor submitted a form, but now they're saying another form is needed. I haven't received any payment in 4 weeks and I'm not sure what form they actually need. Has anyone else dealt with this? I missed their call earlier and now I'm worried about getting through to them again. Any advice would be appreciated!

Yep, for SDI you might need form DE 2501. Make sure your doc filled it out completely. One missing checkbox can delay everything!
