EDD disability claim form delay - due date approaching, need advice ASAP!

Hey everyone, I'm having some issues with my disability claim and I'm hoping someone can help me out. I had surgery last month and filed the form that EDD gave to my doctor. They said they'd send a different form to my doctor, but nothing's shown up yet. The due date is coming up fast and I'm worried it won't get there in time. Has anyone else dealt with this? Any advice on how to move things forward?

I went through something similar a few months ago! Here's what worked for me: I called my doctor's office and asked them to call EDD directly to confirm they were expecting the form. Turns out EDD had the wrong fax number on file. Once my doc's office got the correct info, the form came through within 2 days. Also, if you're running out of time, you can request an extension through your online EDD account - there's usually a "request extension" option under the claim details.
