Where to get ny unemployment tax forms? Phone line dead

I need to file my taxes but can't find my unemployment tax forms anywhere. Been searching for my 1099-G form since February and nothing's showing up on my online account. Tried calling the ny unemployment office every day this week but either get disconnected or stuck in an endless loop of automated messages. The website keeps sending me back to the phone number that doesn't work! Anyone know how to actually get these tax documents? My accountant is getting really annoyed and I'm afraid I'll miss the filing deadline.

You can request your 1099-G through your online account. Log into your ny.gov account, go to My Online Services, then Unemployment Services, and look for the 1099-G request option. It wasn't obvious for me either but it's there. If you still can't find it, I've heard some people had success emailing 1099G@labor.ny.gov with your full name, last 4 of SSN, and mailing address.

Same thing happened to me! The website is total garbage. Try clearing your browser cache or using a different browser. Worked for me using Firefox when Chrome kept failing.

Not sketchy at all. It literally just calls them for you and connects you when someone answers. After 3 weeks of trying myself with no luck, I was able to talk to someone who fixed my 1099-G issue in 10 minutes.

Weird tip but it worked for me - try requesting it through the automated phone system rather than the website. Call the main number and when it asks why you're calling say "tax documents" and follow the prompts. It let me request a mailed copy and I got it 5 days later.

Just FYI for everyone - if you received unemployment benefits, you MUST include it on your tax return even if you never get the 1099-G. You can't just skip reporting it or you'll end up with bigger problems down the road.

After being laughed at by my accountant for not having the proper forms, I finally found out you can view old 1099Gs by logging into your NY.gov ID, then clicking "View My 1099-G" on the unemployment insurance page. BUT you have to disable any popup blockers or it won't work!

Try a completely different browser or even your phone. The site is incredibly buggy and sometimes works on one device but not another for no apparent reason.

This is y I always request paper copies of EVERYTHING from ny unemployment. Their website has been broken since like 2002 lol. When u initially file always check the box for paper correspondence or ur screwed.

They closed most physical offices during covid and never reopened them. The few that are open require appointments which... you guessed it... you can only get by calling the impossible-to-reach phone number 🙃

I filed my taxes without the form using my bank statements to calculate the amount. My accountant said as long as the amount is accurate the IRS won't care. It's not ideal but if you're up against the deadline that's an option.
