PFL Extension Form Confusion - EDD Says Wrong Form Submitted, NP Can't Find Correct One
Hey everyone, I'm having trouble with my PFL extension. The doctor/NP submitted a form, but EDD says it's the wrong one. Now my NP can't find the correct form. It's so confusing and I've been waiting forever. Anyone else deal with this? Any advice on how to get the right form or what to do next?

Ugh, I feel your pain! 😫 The EDD system is a total nightmare. I had a similar issue with my SDI claim. Have you tried calling them directly? Sometimes talking to a real person can help sort things out faster.

I found a way to actually get someone on the phone! I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/DOLxZQb92wM

Here's what you need to do: 1. Go to the EDD website and download form DE 2501F (Claim for Paid Family Leave (PFL) Benefits). 2. Have your healthcare provider fill out the medical certification portion. 3. Make sure ALL sections are completed, including your employer's info. 4. Submit the form online through your EDD account or mail it in. 5. If you're extending an existing claim, you might need form DE 2580EF (Notice of Eligibility and Claim for Extended Paid Family Leave Benefits). Hope this helps! Let me know if you have any questions.

Quick question - are you sure its a PFL extension and not transitioning from SDI to PFL? Those require different forms and its easy to mix them up.

Protip: Take screenshots of EVERYTHING you submit online. EDD has a habit of 'losing' documents. CYA always!
