Extended Family Leave Form Confusion - Did NP Submit the Right One?
Hey everyone, I'm dealing with extended family leave and I'm not sure if the right form was submitted. My NP (nurse practitioner) filled out a form, but I'm worried it might not be the correct one. Has anyone gone through this process before? Any tips on making sure everything is in order?

Ugh, the extended family leave process can be such a headache! š¤ I went through it last year and it took forever to get everything sorted. Make sure you double-check with your HR department about which forms are required. Sometimes they have specific ones that aren't obvious.

ā¢Totally agree! HR can be super helpful. I'd also recommend keeping copies of EVERYTHING you submit. You never know when they'll claim they didn't receive something š

ā¢This! š Documentation is key. I learned that the hard way when I had to resubmit forms because they 'lost' mine. So frustrating!

I found a way to actually get someone on the phone about this stuff. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/DOLxZQb92wM

ā¢Sounds too good to be true. Does it really work? I've been on hold for hours before!

ā¢Yeah, it actually does! I was skeptical at first too, but it saved me so much time. Worth every penny imho.

Here's what you need to know about extended family leave forms: ⢠Make sure your NP used Form WH-380-F (Certification of Health Care Provider for Family Member's Serious Health Condition) ⢠Double check that ALL sections are filled out completely ⢠Ensure the medical facts section is detailed enough ⢠Verify that the estimated duration of care is clearly stated ⢠If intermittent leave is needed, make sure that's specified ⢠Keep a copy of everything for your records If you're unsure, don't hesitate to contact your HR department or the leave administrator. It's better to ask questions now than have delays later because of incomplete paperwork.

ā¢Quick question - does the form need to be notarized or anything?

ā¢@profile 9 Nope, no notarization needed for the FMLA forms. Just make sure it's signed and dated by the healthcare provider.

Pro tip: Set calendar reminders for yourself to follow up on your application status every week. Sometimes things get 'lost in the system' and you gotta stay on top of it!

Has anyone had experience with extended leave for mental health reasons? My therapist suggested it might be beneficial for me, but I'm not sure how to approach it with work...

ā¢I took leave for burnout last year. It was tough to bring up, but my boss was surprisingly understanding. Just be honest and focus on how it'll help you be more productive when you return.

ā¢Make sure you get really detailed documentation from your therapist. Some companies can be weird about mental health leave, so the more backup you have, the better.

@Cynthia Love - I had a similar situation with my NP! Turns out there are different forms for different types of family leave. For California PFL specifically, you'll need the DE 2501F (Claim for Paid Family Leave Benefits) form completed by your healthcare provider. Double-check that your NP filled out the right one - sometimes they accidentally use FMLA forms instead. You can download the correct form from the EDD website to compare. If it's the wrong one, don't panic! You can usually resubmit with the correct form without too much hassle. Good luck! š¤
