EDD Maternity Leave Claim: Tips for Medical Provider Form?
Hey everyone, I just got a medical provider form from the EDD for my maternity leave claim. They're asking my OB to fill it out. Has anyone gone through this process before? Any tips on making sure it goes smoothly? I'm a bit nervous about delays.

I went through this last year. Make sure your OB fills out EVERY section completely. Even if something doesn't apply, have them write N/A. I had mine sent back twice because of blank sections π

I found a way to actually get someone on the phone at EDD. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
