Hey OP, just a heads up - even if you don't have the form, you still need to report the income. You can use your bank statements to estimate the amount received. Better to overestimate slightly than underreport. You can always file an amended return later if needed.

I work in HR and deal with this stuff all the time. Here's what you need to do: 1. Call the PFL office ASAP. The sooner you correct it, the better. 2. Have your claim number, correct start date, and any other relevant info ready. 3. Explain the situation calmly and clearly. It's a common mistake. 4. Ask them to confirm the change verbally, then request written confirmation. 5. Follow up via email or their online portal if they have one. 6. Keep all documentation related to this correction. Don't stress too much. These things happen and they're usually pretty good about fixing them. Just stay on top of it and you should be fine!
