SDI tax forms for December earnings missing - what to do?

Hey everyone, I'm in a bit of a pickle. I haven't gotten my tax forms for my state disability earnings from last December. Anyone else dealing with this? What should I do? I'm worried about filing my taxes without all the proper documentation.

I had the same issue last year. Here's what worked for me: 1. Double-check your mailing address on file with the state disability office. 2. If that's correct, request a duplicate form. You can usually do this online. 3. If all else fails, you can estimate your earnings based on your last payment stub and file your taxes with that. Just be sure to amend later if the numbers are off. Guys, I found a method to actually get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Hey OP, just a heads up - even if you don't have the form, you still need to report the income. You can use your bank statements to estimate the amount received. Better to overestimate slightly than underreport. You can always file an amended return later if needed.
