EDD Parental Leave: How to get Authorization for Release of Records form?
Hey everyone, I'm trying to figure out how to get the Authorization for Release of Records form for my wife's parental leave claim with EDD. We both have claims and I want to check on the status, but I'm having trouble finding the actual form. The EDD website only seems to have a sample. Has anyone successfully obtained this form recently? Any tips on where to look or who to contact?

Ugh, the EDD website is such a maze! š¤¦āāļø I had the same issue last year. Try calling their main line and specifically ask for the Authorization for Release of Records form. They should be able to email it to you or give you a direct link.

I found a method to get someone on the phone quickly. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Have you tried checking your EDD online account? Sometimes they upload forms there that aren't easily findable on the main website. Worth a shot!

I've been through this rodeo before, so here's the deal with the Authorization for Release of Records form: ⢠It's NOT available on the public EDD website ⢠You need to request it directly from an EDD representative ⢠Call the EDD's main line: 1-800-300-5616 ⢠Be prepared for a long wait (seriously, it can be hours) ⢠Once you get someone, ask specifically for the "Authorization for Release of Records" form ⢠They can email it to you or mail a physical copy ⢠Make sure to fill it out completely and follow submission instructions carefully Pro tip: If you're having trouble getting through on the phone, try calling right at 8:00 AM when they open, or late in the afternoon around 4:00 PM. Sometimes the wait times are shorter then. Hope this helps! Good luck with your claims. š

ā¢From my experience, you each need your own form. EDD treats each claim separately, even for married couples.

Hey OP, just a heads up - make sure you and your wife stay on top of any deadlines for submitting docs. EDD can be real sticklers about that stuff. Don't wanna see you guys miss out on benefits cause of some bureaucratic bs!
