SDI Form 2500A: Can't find online, where to mail paper form?
I'm having trouble finding the 2500A form online. The paper copy I have says to go online for faster processing, but I can't seem to locate it anywhere on the website. Has anyone else run into this issue? If I can't submit it online, does anyone know where I should mail the paper form?

I found a way to get someone on the phone about this. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
