Can't print SDI physician certification form online - help locating printable DE 2501 form?
My husband needs to file for disability but we're having technical issues. The doctor is ready to complete his part of the medical certification, but I can't figure out how to print the forms from the EDD website. When I log into SDI Online, it says I can print the application, but I've been clicking everywhere and can't find where to actually download or print the DE 2501 form. I know the doctor needs to complete Part B, but I need the physical form first! Has the website changed recently? I'm getting really frustrated and my husband's condition is getting worse. Any help would be REALLY appreciated!

u dont need to print anything, just have ur husband register for SDI online and the doctor submits their part electronically. much easier that way. my dr did it last month no prob

•But what if his doctor isn't set up for the electronic system? He's been with the same doctor for 20+ years and they're a small practice. I really just need the paper form as a backup.

The paper form you're looking for is called the DE 2501 (Claim for Disability Insurance Benefits). You can download it directly from this page: https://edd.ca.gov/en/disability/How_to_File_a_DI_Claim_by_Mail Scroll down to the "How to file by mail" section and you'll see the PDF link. You'll need to print both Part A (for your husband) and Part B (for the doctor). Just be aware that filing by mail typically takes longer to process than filing online. If possible, I still recommend trying to file electronically.

i had this EXACT same problem last year!!! the EDD website is literally the worst design ever. why make things so complicated??? i ended up having to call them to figure it out and wasted like 3 hours on hold just to get a stupid form. ridiculous.

They get you connected to an actual EDD representative without the endless waiting. Just to clarify some confusion in this thread - there are two ways your husband can file for SDI: 1. ONLINE (fastest): He creates an SDI Online account, completes his portion, and the system generates a receipt with a unique doctor's certificate number. Give this number to the doctor, who then submits their certification electronically. 2. PAPER (slower): Use the DE 2501 form that was linked above. Your husband completes Part A, then physically takes Part B to the doctor to complete. Then mail the entire completed form. Many doctors now prefer the electronic method, but either way works. Just make sure your husband doesn't wait too long to file - claims must be submitted within 49 days of the disability beginning or he may lose benefits.

•It's 49 days from the date your disability begins (the first day you were unable to work due to your condition). Your cousin should file ASAP even if it's been longer than 49 days - he'll need to include a "late statement" explaining the good cause for filing late. Valid reasons can include physical/mental inability to file, believing you were covered by another program, or receiving incorrect information about filing. The doctor's certification will need to cover the entire period of disability.

I ran into this exact issue last month when trying to help my mom file for disability! The EDD website is SO confusing. What worked for us was using Google Chrome instead of Safari - for some reason the print button showed up properly in Chrome. Also make sure you're not blocking pop-ups because sometimes the form opens in a new window that gets blocked. Hope that helps for anyone else struggling with the technical aspects!

His doctor's office said they can do either, but they seemed more comfortable with the paper form (they're not very tech-savvy).

When I had surgery last year my dr office gave ME the form to fill out!! Then I found out THEY were supposed to do it and send it in! Whole thing was a mess and delayed my payments for weeks. Make sure ur doctor knows what they're doing with the SDI paperwork!!

One more important tip - when your husband files his claim, make sure he enters the first day he became disabled accurately. This establishes his benefit year and affects the waiting period (first 7 days aren't paid). Also, if he's received any wages or sick pay during the disability period, he needs to report that on the claim form. Failing to report this correctly is one of the most common reasons claims get delayed or result in overpayment notices later.
