Need to submit paper Part B form after filing EDD SDI online application - where do I send it?
I'm so confused about what to do with my doctor's paperwork! I submitted my initial SDI disability claim online last week through SDI Online, but my doctor filled out Part B (the medical certification) on paper because their office said they don't do the electronic submission. Now I'm not sure where to send this form or who to give it to? Does anyone know if I should mail it to a specific EDD address? Or take it somewhere in person? The online system doesn't give me any instructions about what to do when part of your application is digital and part is on paper. My benefits start date is coming up soon and I'm worried about delays if this form doesn't get to the right place!

You need to mail it to EDD! Same thing happened to me when my doctor refused to do the electronic form. There's an address on the bottom of the paper form where you send it. Make sure you write your claim ID number on it so they know which online application to connect it to.

Hi there! You need to mail the paper medical certification to the SDI office. The mailing address should be on the bottom of the DE 2501 Part B form your doctor completed. Make sure your name and Social Security number are on the form, and I recommend writing your claim ID from your online application at the top too. When I did this in 2025, I also made a photocopy of the form before sending it just to be safe. The address is typically: State Disability Insurance P.O. Box 989311 West Sacramento, CA 95798-9311 I recommend sending it via certified mail so you can track it. It usually takes 1-2 weeks for them to process the paper form after they receive it.

•this is BAD advice!! i tried mailing my form 2 the sacramento address & my claim got delayed for WEEKS. u should take it 2 ur local EDD office instead - they scan it right there & ur claim processes faster!!!

The problem is that SDI is completely separate from the regular EDD unemployment offices that people think of! Taking the form to a local "EDD office" usually means the Unemployment Insurance office, which doesn't process SDI forms. Those forms just sit there or get forwarded with delays. You MUST mail it to the correct SDI P.O. Box that @DisabilityHelper mentioned above. Make sure to include your SSN and claim ID number on every page! Also, if you're worried about timing, call the SDI office to let them know you've mailed in the form. They can make a note on your account. I've dealt with this situation countless times working with clients dealing with disability claims. The mail-in process is frustrating but unavoidable when doctors don't use SDI Online.

•Thank you for explaining! I'll definitely mail it to the PO Box address. My doctor is a small practice and said they just don't have the staff to figure out the online system. Do you think I should call EDD first or just mail it and then call to confirm they received it?

i just went thru this whole thing last month!! omg the stress!! make sure u COPY EVERYTHING before u mail it!!! my doctor's form got "lost" and i had to get him to fill out a second one which delayed my payments by THREE WEEKS!!! i was about ready to scream. definitely use certified mail with tracking!!

Also did anyone else have trouble with the doctor filling out the wrong dates on the form?? My doc put the wrong disability start date and it took forever to get it fixed. Double check all the dates before you mail it in!!

Yes, absolutely have your doctor correct the dates if they don't match your online application! Mismatched dates are one of the most common reasons for claim processing delays. The start date on your online application and your doctor's certification should be identical. If you've already filed online with a specific date, your doctor's certification needs to match that date. Otherwise, EDD will flag your claim for review, which can add 2-3 weeks to your processing time. Update: I got the dates corrected and mailed in the form yesterday with certified mail! Also managed to get through to EDD after using the Claimyr service (it actually worked), and they confirmed that mailing to the PO Box was the right move. They said once they receive it, they'll match it to my online application and continue processing my claim. Agent said to allow 7-10 business days from when they receive it. Once your claim is fully processed, you'll see your claim status change from "Pending" to "Active" and you should see information about your weekly benefit amount.
