Can I upload paper extension form to EDD SDI or must it be mailed? Doctor took forever!
I'm stressing out about my SDI extension paperwork! My doctor finally completed my supplemental certification form for extending my disability benefits (took him almost 2 weeks!), but he only does paper forms - refuses to do electronic submission. Now I'm holding this completed form and not sure what to do with it. There's no fax number listed on the form. Can I scan and upload it through SDI Online somehow, or do I HAVE to mail it? I'm worried about delays if I mail it, but also concerned that uploading might cause confusion in the system since it was originally sent to my doctor as a paper form. Has anyone dealt with paper extension forms recently? What's the fastest way to get this processed without messing up my claim?

i had same issue last month, u HAVE to mail it. they dont have upload for those forms. mine took like 9 days to process after i mailed it

Actually, there IS a way to upload the paper form! Go to your SDI Online account, click on 'Contact EDD' at the bottom, then select 'Submit Question' from the dropdown menu. Choose 'State Disability Insurance' as the category, then attach your scanned form as a PDF or image file. Include your claim number in the subject line and explain that you're submitting a supplemental certification form for extension. I did this with my last claim in January and they processed it within 3 days. Just make sure you scan it clearly and all information is legible.

My doctor also only does paper forms (SO annoying!) and when I called EDD about this they told me to mail it to the PO Box on the form. But that was back in 2023 so maybe things changed?

I HATE how doctors still refuse to do electronic submissions!!! It's 2025 for crying out loud! My doctor made me wait 3 weeks for my initial paperwork and almost ruined my claim. Make sure whatever you do you keep a copy of that form and proof that you submitted it! My husband's extension was "lost" twice and we had to resubmit. now i take pictures of everything before sending.
