Medical provider paper form for EDD SDI - does it get mailed automatically?

I just filed a new SDI claim about 5 days ago, and my claim status is showing 'Pending medical provider form' when I log into SDI Online. My doctor prefers doing paperwork the old-fashioned way instead of electronically, but I can't remember from my previous claim whether EDD automatically mails out the form to them or if I need to take some action. Do I need to download and print the form myself to give to my doctor? Or will EDD mail it directly to my provider based on the info I submitted in my claim? I'm worried about delaying my benefits if I'm missing a step here. Thanks for any help!

The medical certification form (DE 2501) doesn't automatically get mailed to your provider. If your doctor prefers paper forms, you need to either print the form from the SDI Online portal and take it to them, or have your doctor's office download it directly from the EDD website. There should be a link in your SDI Online account under 'Forms' that lets you print the medical certification portion.

my dr office said they never get the forms in mail anymore, edd stopped doing that in like 2019 or 2020. u gotta bring it to them urself. good luck gettin ur money soon!!

I had the EXACT same issue 4 months ago with my herniated disc claim!!! I waited TWO WHOLE WEEKS thinking EDD was going to mail something to my doctor. Meanwhile my claim just sat there "Pending medical provider form" and no benefits!! I called EDD like 67 times before I finally got through and they said I needed to print the form myself. Actually, I think they CAN still mail the form in certain circumstances, but you shouldn't count on it. If you indicated on your initial application that your doctor prefers paper forms, there's a chance EDD might mail it, but it's always faster and more reliable to print it yourself and deliver it to your doctor. The key is making sure the form has your correct claim ID number on it so it gets properly matched to your file when they submit it back to EDD.

yes this is correct! i work at a medical office and we do occasionally still get forms mailed from EDD but its super random and we never know which patients will get mailed forms. we always tell patients to bring the form themselves to be safe.

dont forget that after ur doctor fills out the form, THEY have to submit it back to EDD, not you! i made that mistake and carried the completed form around in my purse for a week wondering why my claim wasnt moving forward lol

yep! they should know this but sometimes the front desk staff forgets. they can either mail it to the address on the form or fax it. some offices scan and upload it too.

One more important point - make sure to follow up with your doctor's office within a week after they have the form. Medical offices get busy and these forms can sit on someone's desk. The sooner they submit it, the sooner your claim moves from 'pending' to 'processing' and then to 'paid'. Your doctor has to certify your disability before EDD will release any payment.
