EDD not receiving Physician/Practitioner Supplementary Certificate - What to do?
Has anyone else had trouble with EDD not receiving their forms? I sent in my Physician/Practitioner Supplementary Certificate over a week ago and the post office says it was delivered, but EDD hasn't confirmed they got it. I'm really worried because if they don't get this form, my benefits could be cancelled. Anyone know how long it usually takes for them to process these forms? Or what I should do if they never received it?
