Not to be that guy, but have you double-checked that you sent it to the right address/fax/whatever? I once spent weeks waiting only to realize I messed up the address 🤦‍♂️

Idk if this helps, but I had a similar issue and it turned out my doctor hadn't filled out the form completely. Might be worth checking with them to make sure everything was filled out correctly.
