DEO wage determination missing 2019 income - where to upload proof?

Got my monetary determination letter yesterday and DEO is missing like half my income from 2019! I worked at two different companies that year but they only show one. The determination says I can submit proof of the missing wages but there's NOWHERE on the CONNECT site that I can find to upload these documents?? I've got my W2 and paystubs ready to go. Has anyone dealt with this before? Where exactly on the site am I supposed to upload this stuff? The clock is ticking and I'm worried about losing benefits I'm entitled to because of their mistake. HELP PLEASE!

omg same thing happened to me last month! they didnt have my gig work from 2019 at all. so frustrating. i think u have to call them directly, i couldnt find any upload option either.

You need to submit a Request for Reconsideration of Monetary Determination. There isn't a direct upload option for this in CONNECT. You have to: 1. Fill out form DEO-UCB-13 (Wage Credits Investigation Form) 2. Mail it along with your proof of income (W2s, paystubs) to: Florida Department of Economic Opportunity P.O. Box 5350 Tallahassee, FL 32314-5350 Make sure to include your claimant ID number on everything. They have 21 days to process it once received, but it can take longer.

Make sure to keep copies of everything you send them and consider using certified mail so you have proof of when it was delivered.

OMG it took like 6 weeks!! And that was AFTER they received my paperwork. The worst part was not knowing if they were even looking at it.

You still need to submit the form with your wage evidence, but talking to someone helps in two ways: 1) They can confirm they received your documents, and 2) They can sometimes expedite the processing if you explain your situation. The agent I spoke with actually flagged my case for faster review after I explained I was facing eviction.

Quick question - did they spell your employer's name correctly on the determination? Mine was misspelled and apparently that was causing the wage issue. Just something to check.

Just checked and everything looks spelled correctly. The issue is that my second employer from 2019 isn't showing up at all. It's like they have zero record of me working there.

Something to consider - if your second employer was out of state or didn't properly report your wages to Florida's unemployment system, you'll definitely need to provide that documentation. Also, the wage base period for 2025 claims is a bit different - they look at Oct 2023 - Sept 2024 for base wages, not 2019. Are you sure 2019 is the relevant year for your claim?

Oh wait - you're right! I just double-checked my determination letter and they're looking at 2023-2024, not 2019. I've been stressing about the wrong thing! Need to check those quarters instead. Thank you for pointing this out!

Glad I could help! DEO's letters can be really confusing. Make sure you look at the Base Period box on your Monetary Determination letter - it shows exactly which quarters they're using to calculate your benefits. Then check if all your employers during those quarters are listed correctly with the right wage amounts.
