Told about disqualification by phone but never received EDD appeal form - need address ASAP

I just got blindsided during a phone call with an EDD rep who informed me I've been disqualified from receiving benefits (something about insufficient wages in my base period). They said I could appeal but I needed to submit a written appeal. Eventually, but only after I got my assemblyman's office involved. Took almost 4 months and I had to borrow money from family to avoid eviction. The whole system is designed to make people give up.
