Received a random notice from NY unemployment but I never filed - identity theft?

So yesterday I got this surprise letter from the NY unemployment office saying my claim was approved for $470 weekly benefits starting in June. The problem is I never filed for unemployment! I'm currently employed full-time at the same company I've been with for 3 years. I'm freaking out because someone clearly stole my identity and filed a fraudulent claim. I immediately called my employer who confirmed they received a notice about this claim too. I've been trying to reach the unemployment office for two days straight but can't get through to anyone. The automated system just disconnects me after 45 minutes on hold. I've sent emails through their contact portal but no response yet. I also filed a police report and put a freeze on my credit. Has anyone dealt with this before? What else should I do? I'm worried this will affect my taxes or current employment somehow.

Same thing happened to me in September! Someone filed using my info while I was still employed. What worked for me was going to the fraud section on the ny unemployment website and filling out their identity theft form. They have a specific form for this exact situation. Takes about 3-4 weeks for them to close the fraudulent claim. Also notify your HR department so they can contest it from their end.

Nope, never found out. The unemployment office just closed the claim and sent me a letter saying it was marked as fraud. My employer told me this happens alot lately.
