DE2501F Form: Do I need to put SSN on every page for PFL bonding claim?
Hey everyone, I'm filling out the DE2501F form for Paid Family Leave and I'm confused about the social security number requirement. Do I really need to put it on every single page, even the ones that don't apply to my bonding claim? And what about the attached documents like the hospital verification letter? Should I include it there too? I'm especially unsure about Parts C, D, and E since they're not relevant to my situation. Any advice would be appreciated!

I just went through this process! Here's what I learned: β’ Yes, put your SSN on every page of the DE2501F form, even if the section doesn't apply to you. β’ For attached documents like hospital letters, it's best to include your SSN as well. β’ For Parts C, D, and E that don't apply, still write your SSN at the top of those pages. β’ If there's no designated spot for SSN on a page, write it in the top right corner. Just wanted to add my experience - I completed my DE2501F form last month and yes, you absolutely need your SSN on every page! I initially skipped it on the pages that didn't apply to me and got a letter back asking me to resubmit with SSN on ALL pages. Learned that lesson the hard way π For the hospital verification letter, I wrote my SSN in small print at the top corner just to be safe. Better to over-document than have delays in processing. The whole thing took about 3 weeks total once I resubmitted properly. Hang in there!
