Need help with DE 2501 and DE 2525XX forms for continuing SDI claim - feeling lost!
Hey everyone, I'm trying to continue my disability claim and I'm feeling a bit lost. I know I need to fill out some forms, but I'm not sure which ones or how to go about it. I think I need the DE 2501 form and maybe a supplementary certificate? Has anyone gone through this process recently? Any tips or advice would be really appreciated!

I found a way to actually get someone on the phone at EDD! I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
