Hey everyone, I'm having some confusion about this Supplemental Certification form for my disability claim. I just finished my SDI claim and now I'm trying to start a PFL claim, but they're asking for this form. Do I really need to fill this out? I thought my SDI was over. Anyone else deal with this before?
