What form does my doctor need for EDD SDI extension? DE 2525XX?

I've been on SDI for almost 8 weeks now and my doctor says I need more time to recover. She asked me to find out what form she needs to fill out for my disability extension. Is it the same form as the original certification? Something with numbers like DE 2525 or something? The EDD website is so confusing and I can't get through on the phone. I have an appointment with her next Tuesday and need to bring the right form. Thanks for any help!

Your doctor needs to submit the DE 2525XX form. Just to be clear, YOU don't submit this form - your doctor does. They can send it electronically through SDI Online or mail it. If your doctor doesn't have access to the form, you can provide them with the extension form, but they're responsible for completing and submitting it. When I needed an extension it was IMPOSSIBLE to get through to EDD! Called for DAYS and either got disconnected or was on hold FOREVER!!! Finally used this service called Claimyr (claimyr.com) which got me connected to an actual EDD agent in about 20 minutes!!
