Help with PFL form: Questions about other pay, verification, and required documents
Hey everyone, I'm filling out my paid family leave form for the first time and I've got a few questions. I'm trying to avoid delays in my application. Can anyone help me out? 1. Why are they asking about other pay I received during my time off? I marked sick pay for COVID, but the time I'm requesting for pay starts after that period. 2. What's the deal with the last page before verification? Is there supposed to be a checklist or something? 3. After I finish, will I get instructions on how to upload Part C of the verification portion? I think Part D gets turned in for me. 4. Do I need to include any other documents like a W-2 or pay stub? Any advice would be really appreciated!

I went through this recently, so I can help you out! 1. They ask about other pay to make sure there's no overlap in benefits. Just make sure your PFL dates don't overlap with your COVID sick pay dates. 2. The last page is usually just a final review. I don't remember seeing a specific checklist, but double-check when you have the form in front of you. 3. Yes, you'll get instructions for uploading Part C after you complete the form. Have it ready to go. 4. Usually, you don't need W-2 or pay stubs unless they specifically ask for them. Just submit the forms as requested. Hope this helps! •It took about 2 weeks for mine to be processed, but I've heard it can vary. Some people get it faster, others wait longer. Depends on how busy they are, I guess.

Hey, I found a way to actually get someone on the phone to answer these questions! I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Pro tip: take screenshots of EVERYTHING as you fill out the form. You never know when the system might glitch and you'll need to prove what you submitted. Learned that the hard way ðŸ˜
