I applied for SDI for pregnancy leave and the status is showing that my provider needs to send a complete medical form. I shared my receipt number with my provider and they sent it over 2 weeks ago. Now I'm confused because I also received a paper form from SDI for my provider to fill out. Does anyone know what's going on or what I should do next? I'm not sure if they need to fill out another form or what the next steps are in this process.
