SDI form never arrived after filing claim for maternity leave - What to do?

Hey everyone, I'm having some issues with my disability claim after having a baby. I filed for State Disability Insurance (SDI) and they told me they'd send a form, but I never got it. Has anyone else had this problem? What should I do now? I'm getting worried about when I'll actually get paid.
