Need DE 1000A appeal form for SDI claim - EDD didn't include it, can't reach anyone by phone. Help!

I've been trying to get a DE 1000A appeal form for my SDI claim, but it wasn't included in my Notice of Determination. I spent over an hour trying to reach someone on the phone with no luck. Has anyone else dealt with this? Any tips on how to actually get through to a human at EDD?

I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
