SDI claim denied - DE 1000A form missing from Notice of Determination envelope. Help!
I need help with my SDI claim. I got a Notice of Determination but they didn't include the DE 1000A appeal form in the envelope. I can't move forward with my claim without it. When I called, they said they couldn't identify my doctor. Has anyone else dealt with this? What should I do next?

Ugh, that sucks. The EDD is such a mess rn. I had a similar issue last year. Have you tried faxing them? Sometimes that works better than calling.

I found a way to actually get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/1X-mEsLtbmQ

Have you checked your spam folder? Sometimes EDD emails end up there. Also, make sure your address is up to date in their system.

I went through this exact same situation last month! The missing DE 1000A form seems to be a common issue. Here's what worked for me: I called the main EDD number early in the morning (like 8:05 AM) and when I finally got through, I explained that the appeal form was missing from my Notice of Determination envelope. They were able to email me the form directly and also helped me understand why my doctor wasn't being identified in their system (turns out there was a mismatch in the way the medical office submitted my paperwork). Don't give up - you have appeal rights and they have to provide you with the proper forms!
