Which tax pages needed for NY unemployment 2019 proof of work deadline?
I just got a notice saying I need to provide proof of work from 2019 by June 1, 2025. I'm confused about exactly which pages of my 2019 tax returns I need to upload to the NY unemployment website. The notice mentions either tax documents OR paycheck stubs, but I'm thinking maybe I should just upload both to be safe? Has anyone done this recently? I don't want to miss anything and lose my benefits because I didn't submit the right pages.

For the 2019 proof of work requirement, you should definitely submit your full 2019 tax return - including ALL schedules that were filed. At minimum, you need Form 1040 and Schedule C if you were self-employed. If you were a W-2 employee, include your W-2 forms too. As for submitting both tax returns AND pay stubs - yes, that's actually a good strategy. The more documentation you provide, the better your chances of approval. Just make sure everything you upload is clear and readable.

•Thank you! So I need the full 1040 plus all the schedules? I was only going to send the first two pages. Also, should I include my W2s even though they're already part of my tax return?

•Right? I'm worried about the deadline. Do you know if there's any way to get an extension if I can't find all my documents in time?

When I submitted mine last month, I sent EVERYTHING I could find - all tax pages, W2s, even my last paystub from 2019. I was so worried they'd deny me! The system let me upload multiple files though. Just make sure none of your files exceed the size limit (I think it's 4MB per file?). I had to take pictures of some documents with my phone because my scanner wasn't working.

For the NY unemployment proof of work requirement, you need to submit the following tax documents from 2019: 1. Form 1040 (all pages) 2. Schedule 1 if applicable 3. Schedule C if you were self-employed 4. Schedule SE if you paid self-employment tax 5. Any W-2 forms They need these documents to verify your income sources and employment status. Including both tax documents AND pay stubs is actually recommended by NYSDOL representatives. The system allows multiple uploads, so there's no downside to providing extra documentation.

•This is really helpful, thank you! One more question - do I need to highlight or circle anything on the documents? The notice wasn't clear about that.

No need to highlight or circle anything. The claims examiners know exactly what they're looking for on each form. Just make sure all pages are complete, clearly scanned/photographed, and that your name and SSN are visible. Also important: save your confirmation number after uploading! Take a screenshot of the confirmation page too. This is your proof that you submitted documents on time if there's ever a question later.

•YES! Same thing happened to me. I ended up trying at like 11pm and it finally worked. I think their servers get overloaded during regular hours with everyone trying to submit at once.

After struggling for DAYS to get through on the phone to ask this exact question, I finally used Claimyr (claimyr.com) to get connected to an actual NYSDOL agent in about 20 minutes. The agent told me that for the 2019 proof of work, they specifically need ALL pages of Form 1040 plus any schedules that show your income. For W-2 employees, they also want to see your W-2 forms. If you're also having trouble reaching someone by phone to get answers before the deadline, check out their demo video: https://youtu.be/Rdqa1gKtxuE - it saved me hours of redial frustration. The agent I spoke with confirmed that submitting both tax documents and pay stubs is actually the safest approach.

•This is really helpful, thank you! I've been trying to call for days with no luck. I'll check out that service because I have a few more questions I'd like to ask a real agent before submitting everything.

•Don't panic! You can get a tax transcript directly from the IRS website. Go to IRS.gov and search for "Get Transcript Online" - you can download your full 2019 transcript immediately. If you filed with a tax preparer like H&R Block or TurboTax, you can also log into your account there and download your full return.

Update: I just submitted all my documents! I ended up sending the full 1040 with all schedules, plus my W-2s, and several pay stubs from different parts of 2019. The upload process was actually easier than I expected. Thanks everyone for the help!

•Congrats! Did you get a confirmation email or just the number on the screen? I submitted mine but never got an email and now I'm worried.

I just got the confirmation number on the screen - no email. I took a screenshot just to be safe. Maybe check your spam folder though?
