How to get proof of NY unemployment end date for verification purposes?
I need documentation showing when my NY unemployment benefits ended but I'm hitting a wall trying to get it. I've tried calling the unemployment office multiple times (always busy or disconnected), left messages through my online account, and still nothing after 2 weeks. This is urgent since I need it for a housing application that's due next week! Does anyone know the correct department to contact or a specific form to request this kind of documentation? The regular customer service doesn't seem to be working for me.

You need to request an official 'Record of Benefits' letter. Log into your NY.gov account, then go to your unemployment dashboard. Look for the 'Documents and Forms' section, then select 'Request Benefit History'. Make sure you select the option that includes benefit end dates, not just payment history. There's actually a specific form for this: it's called the "Request for Unemployment Insurance Records" (Form UI-999P). You can download it from the NYSDOL website, fill it out, and upload it through your account's document portal. Be very specific about what dates you need verified. Check box #3 which is for "Benefit End Date Verification" specifically. Make sure you include your full name, the last 4 of your SSN, and your NY.gov ID on the top of the form or it will get rejected (happened to me twice).

•THIS IS WRONG INFO!! There's no such thing as form UI-999P! I wasted 3 days looking for this nonexistent form!!! The correct form is actually called REQUEST FOR BENEFIT PAYMENT HISTORY (LO430) and you have to specifically write that you need END DATE verification not just payment history!!! Why do people just make stuff up???

After trying for WEEKS to get through to NY unemployment about a similar document (in my case for mortgage verification), I finally used Claimyr to connect with an agent. It got me through to someone in about 20 minutes when I had been trying for days with no luck. Just went to claimyr.com and used their service to get connected directly. The agent I spoke with emailed me the benefits letter within an hour after our call. Worth trying if you're in a time crunch for that housing application.

If the online method I suggested earlier doesn't work, you can also try contacting the Records Access Office directly at 518-457-6603. They specifically handle document requests like this. When you call, ask for a "Statement of Benefits" with end date verification. They can email it to you within 1-2 business days if you explain it's urgent for housing. Make sure to have your NY.gov ID and PIN ready when you call.

I needed the same thing last yr for a mortgage. Honestly the FASTEST way is to reach out to your state assembly person's office!!! I was getting nowhere with NYSDOL for WEEKS then contacted my local assembly member's constituent services and they got me the letter in 3 DAYS! They have special contacts at these agencies. Google "NY state assembly finder" to locate yours.

OMG the NY unemployment system is SUCH A DISASTER!!! I've been dealing with them for months trying to get simple documentation. Everything is broken and nobody knows what they're doing! Quick update on my earlier comment: it seems they updated their forms recently. The UI-999P was renamed in January 2025 to the LO430 form that someone mentioned above. However, the most efficient method is actually through your NY.gov account under My Documents > Request Official Records > Benefit Verification. This generates an official PDF with a verification code that's legally acceptable for housing applications. I work in HR and we accept these for employment verification regularly.
