Need 1099-G tax form from Florida DEO after moving out of state - can't access CONNECT

I'm panicking about tax season here! I collected Florida unemployment from May 2025 through August 2026 before relocating to Nevada for a new job opportunity. Now I'm trying to file my taxes and realized I never received my 1099-G form from Florida DEO. My tax forms haven't arrived at my Nevada address, and when I tried logging into my CONNECT account, the whole system seems completely different from what I remember! I can't figure out how to access my tax documents online. Has anyone dealt with getting 1099-G forms after moving to another state? The Nevada unemployment office said they can't help with Florida records. I've tried calling DEO but keep getting disconnected after waiting forever. What should I do to get my tax documents?? Filing deadline is coming up!

You can request your 1099-G online or by phone. Go to FloridaJobs.org and look for the '1099-G Tax Form Request' section - you don't need to log into CONNECT for that specific function. They added a separate portal just for tax documents last year. You'll need your SSN, DOB, and Florida address that was on file. If the online portal doesn't work, call the dedicated 1099-G request line at 1-833-FL-APPLY (352-7759) and choose the tax document option. They can mail it to your current Nevada address once they verify your identity.

The DEO is completely USELESS when it comes to anything tax related!!! I've lived in Florida the whole time and STILL had problems getting my 1099-G this year. The new system they implemented is a JOKE and half the time the website crashes when you try to download your forms. I ended up having to file an extension because they couldn't get their act together. Good luck getting anything from another state!!

While the system definitely has its issues, requesting the 1099-G has actually improved compared to previous years. The dedicated portal is separate from CONNECT and works better. Make sure you're going to FloridaJobs.org/1099g and not trying through the main CONNECT system. The phone system has also been upgraded specifically for tax document requests.

My cousin moved to Georgia last year and had this same issue! He said he had to fax (yes, fax lol) a request form to DEO with his new address. Think it took like 3 weeks to get the 1099 after that. So maybe start looking for a place that still does faxing? Kinko's maybe?

Try using Claimyr to get through to a DEO agent. I was having the same issue trying to get my 1099-G after moving to North Carolina and couldn't get through on the phone for days. If you can get through to DEO, they can tell you the exact withholding amount over the phone. UPDATE: Wanted to share what worked for me in case anyone else has this problem! Got connected to a DEO agent who helped me request my 1099-G and confirmed my withholding amount.
