Do I need to mail DE2647DF form after faxing/uploading to EDD for caregiving PFL?

Hi everyone! I'm currently on PFL to care for my husband who's recovering from spinal surgery. EDD sent me form DE2647DF (Request for Medical Information) that needs to be completed by my husband's surgeon. The form shows both a P.O. Box address and a fax number for submission. I already faxed it last Tuesday AND uploaded a copy through my EDD online account yesterday. I've already received the benefit calculation notice showing my weekly amount ($1,473), but I'm worried about the form submission. Do I also need to physically mail the original form? I'm paranoid about delaying my payments since we're really depending on this income while my husband recovers. Has anyone gone through this process recently? What did you do with this form?

You're fine with just the fax and upload - no need to mail it! I went through this exact same situation in January when caring for my mother after her stroke. The physical mail option is just an alternative if you can't fax or upload. Since you've already received your benefit calculation notice, that's a good sign they have the information they need. Just make sure to keep completing your certifications on time every two weeks once they become available.

Actually, I'd mail it anyway if I were you. I lost nearly 3 weeks of benefits because they claimed they never received my husband's medical documentation even though I uploaded AND faxed it. When I finally got through to a rep, they said sometimes the faxed documents don't get properly attached to your claim, and uploads can get lost in their system too. The safest bet is to use all three methods and keep proof of everything. Having that benefit calculation notice doesn't guarantee they received the DE2647DF form - those are sent out automatically based on wage data. Then again, their system is so messed up that you might be fine. Just my two cents after dealing with their incompetence for months!

I'm going through caregiver PFL right now for my dad, and let me tell you - EDD's documentation system is a complete mess. Here's what I recommend based on my experience: 1. Since you've faxed AND uploaded the DE2647DF, you've covered the major bases. 2. Call EDD to confirm they received it - this is the ONLY way to be 100% sure. 3. Keep the fax confirmation and take screenshots of your upload confirmation. The benefit calculation notice doesn't confirm they received the medical documentation - I got that notice before submitting any medical forms. It's just based on your wage history. You won't know if they truly have the doctor's form until your first payment either processes or gets held up. Also, make sure your husband's doctor filled out ALL fields on the form. Missing a single signature or date can delay everything by weeks.

Speaking as someone who processes these forms (I work in a doctor's office), I can tell you that the EDD's preferred method is actually the fax or electronic upload. Physical mail is their least preferred since it requires manual scanning. BUT - and this is important - their systems don't always talk to each other properly. The benefit calculation notice (DE429DF) is automatically generated based on your wage history and has nothing to do with whether they received your husband's medical documentation. That comes later in the process. To be extra safe, I recommend: 1. Keep your fax confirmation sheet 2. Take screenshots of your upload confirmation 3. Call EDD to confirm receipt (though I know this is nearly impossible) If you've already done the fax AND upload, mailing is probably unnecessary, but if you want absolute peace of mind, it doesn't hurt to send it via certified mail too.

omg the EDD is such a disaster!!!! when i was on maternity leav last year they sent me like 5 difrent forms and i had no idea which ones i needed to send back. ended up having my doctor fill out THE WRONG FORM and lost 3 weeks of pay bc of it. so frustrating!!!

The worst part is every rep tells you something different! I got conflicting advice from 3 different EDD agents about the same form. Their training must be nonexistent.

Quick update on my earlier comment - I just checked my notes from January, and I did receive my benefit calculation notice BEFORE they processed my DE2647DF form. So getting that notice doesn't necessarily mean they've processed your doctor's certification yet. However, since you've both faxed and uploaded the form, you should be covered. The most important thing now is to watch for your certification periods to become available and complete those on time.
