SDI won't process my last payment after sending paperwork 3 times - need help!

I'm having trouble getting my last SDI payment. I've had to send paperwork to SDI three times now because of mistakes made by my doctor's office. The last time, I got SDI to mail me the proper certification form. I filled it out and sent it via overnight delivery about two weeks ago. I've been trying to contact them to check on the status, but I can't get through. Has anyone else dealt with this? Any advice on how to make sure they received my form and are processing it correctly?
