Getting a copy of my TTD claim application for doctor verification - ESD won't answer!

I'm in a real bind here and running out of options. I filed for Temporary Total Disability unemployment a couple weeks ago and just received a letter from ESD saying my doctor needs to fill out some verification form. The problem is, my doctor needs to see my original application to verify all the dates and medical info I submitted are accurate. But I can't find my application ANYWHERE on my ESD account and they never emailed me a copy! I've had multiple doctors clear me for work at different times (had a complicated recovery) and honestly can't remember which return-to-work date I put on my application. I've tried calling ESD literally 23 times this week but either get disconnected or stuck on hold forever. Has anyone figured out how to get a copy of your submitted application after the fact? I'm going to start saving EVERYTHING from now on, but that doesn't help me right now. My benefits are on hold until this gets resolved and rent is coming up fast.

You can request a copy of your application through your eServices account. Look for 'Submitted Forms' or 'Documents' in your dashboard. If it's not there, you need to submit a records request through the Contact Us page. Include your claim ID number and specifically ask for a copy of your TTD application. They're required to provide it, but it might take 5-10 business days to get back to you.

Thanks - I looked in eServices but there's nothing under Documents except the determination letter. I submitted a records request yesterday through Contact Us but haven't heard anything back. Do you know if there's a faster way? My doctor's office needs this by next Tuesday or they'll have to reschedule my appointment out another 3 weeks.

same thing happened to me in january!!! its so frustrating, right??? i spent like 2 weeks trying to get thru to someone at esd. did u try calling super early like right when they open? thats what finally worked for me

I've tried early morning, late afternoon, mid-day... no luck getting through. It's driving me crazy! How early did you call? Maybe I need to try 7:55am or something?

i got thru at like 8:02am but i started dialing at 7:58 lol. just kept hitting redial. took me like 6 tries then got in the queue. still waited 40 mins but at least somebody answered eventually!

The ESD system is RIDICULOUS. I've been through this exact nightmare with TTD claims. Here's what really happens: your application gets stored in their system but they don't give you access to view it after submission. It's BY DESIGN so they can deny claims when doctors can't match exact information. I had to threaten a formal complaint with the governor's office before they would email me my application. Even then, took 12 days to get it. The system is BROKEN.

Whoa that seems a bit extreme. I think it's just outdated systems not a conspiracy. I had missing docs too but the WorkSource office helped me, have you tried visiting one in person?

Not extreme, ACCURATE. I worked in state government for 8 years. The systems are designed to create friction points that lead to claim abandonment. About 22% of legitimate claims get abandoned due to these "technical hurdles" - it saves the state millions. WorkSource staff don't have access to your actual application docs, just a summary screen.

I was stuck in this same situation last year with my TTD claim. After weeks of trying to reach ESD, I discovered Claimyr which connected me to an actual ESD agent in about 20 minutes. You need to be very specific. Ask for a "copy of my originally submitted TTD application with all entered information" - those exact words. Make sure to verify your identity when they ask, and explain it's for your doctor to complete the medical certification form. The first agent transferred me to a claims specialist who handled it.

Here's the official process for this situation: 1. If your doctor needs to verify information on your TTD application, they only need to complete the form ESD sent you with their own records of your disability. 2. For a copy of your application, submit a formal records request using form ESD-RECD-01 (available on ESD website under Forms). This is different from a general Contact Us message. 3. Medical certification forms (which I assume is what ESD sent) require only your name, claim ID, disability period, and diagnosis - your doctor should have all this in their records. 4. If there's a discrepancy between dates, your doctor should document YOUR correct dates based on their medical records, not what might be on your application. Records requests are processed in 7-10 business days according to statute. However, if you explain it's for completing required medical certification, you can request expedited processing (2-3 business days). Make sure to check the "urgent" box on the form and include your deadline in the notes. Your doctor can also write "dates under medical review" on the certification form if they're uncertain about matching your application dates exactly.

WorkSource staff don't have access to your submitted forms in the system. They use a different database than the claims center. They might be able to help with general questions but not retrieving documents. I used to work there.

Thanks for the suggestion. There's a WorkSource office not too far from me - I might try that tomorrow if I can't get through on the phone. At this point I'm willing to try anything!

Quick tip: you can sometimes see part of your application info if you start a new claim but don't submit it. The system pre-fills some fields with your last application data. I did this when I needed to check what start date I used for my disability claim. Not a perfect solution but might help in a pinch!

Let me know if it works! It helped me in a similar situation, but I know the ESD website gets updated sometimes so fingers crossed it still works this way.
