I filed a short term disability claim for being out with coronavirus and in the claim it says pending medical provider form but I got my test through the county health department not a doctor. Does anyone have any advice or experience dealing with short term disability due to Covid?

Your doctor would still have to send in their portion. My husband is out with short term disability but not due to covid and those exact words said on his account “pending medical provider form”.
