EDD certification mistake: Listed consulting client as employer instead of self-employed. How to fix?

I messed up big time with my unemployment certification and now I'm in a pickle. I've been doing some part-time consulting work and I listed the company I was consulting for as my employer instead of putting myself down as self-employed. Now the company got a notice from the state and they're freaking out about potential penalties or increased unemployment insurance premiums. I need to fix this ASAP for the past weeks and figure out how to fill out the certification correctly going forward. Anyone been through something similar or know how to handle this without getting myself or the company in trouble?
