Need help with EDD benefit extension form revision by doctor - deadlines and submission process?
Hey everyone, I need some help with a benefit extension form. I have to get my doctor to revise it, but I'm not sure about the process. Does anyone have experience with this? What information do I need from my doctor, and are there any deadlines I should know about? Also, once I get the revised form, what are the next steps for submitting it? Any advice would be really appreciated!

Hey guys, I found a method to actually get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
