How to confirm correct form for doctor? Need help ASAP!
Hey everyone, I'm trying to get the right form for my doctor to fill out. Has anyone gone through this process recently? I want to make sure I have the correct paperwork before I go to my appointment. Any tips on how to confirm it's the right form?

I found a way to actually get someone on the phone to help with these kinda questions. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
