Need help getting physician extension form mailed - online access restricted

Hey everyone, I'm having trouble getting an extension form for my physician. The online instructions say only doctors can access it, but my doctor doesn't have an online account. They need the paper version, but I can't find it anywhere online. Has anyone else dealt with this? Any ideas on how to get the form mailed to me?

I had the same issue last year. Here's what worked for me: 1. Go to the agency's website 2. Look for a 'Forms' or 'Resources' section 3. Search for 'Extension Request' or 'Physician Extension' 4. I found a method to get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video explaining it: https://youtu.be/1X-mEsLtbmQ

Sure! You choose the department you need to reach, pay the fee, and they start calling. When an agent picks up, your phone rings. No guarantee, but it worked for me and others I know.
