EDD form not mailed out as promised - what to do?

Hey everyone, I'm having an issue with a form that was supposed to be mailed to me. It was meant to be sent out at the beginning of this month for my doctor to fill out, but I still haven't received it. Has anyone else experienced delays with important forms being mailed out? Any advice on what I should do next?

I found a method to get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/1X-mEsLtbmQ

Here's what I would do in your situation: 1. Call them first thing in the morning to inquire about the status of your form. 2. Double-check that they have your correct mailing address on file. 3. Ask if there's an option to have the form emailed or faxed to you instead. 4. If they say it's already been sent, request that they resend it and confirm when it's been mailed out. 5. Keep a record of who you spoke to and when, in case you need to follow up again. 6. If possible, see if your doctor's office can request the form directly. Sometimes they have better luck getting through. 7. As a last resort, see if you can go to a local office in person to pick up the form. Remember to be patient but persistent. Good luck!
