NPI issues causing 6-week delay in Part B form submission - need advice!

I'm having a nightmare with my provider trying to fill out my Part B form. They can't do it online because of NPI issues and they put in a ticket weeks ago but still no response. They even tried ordering paper forms but those haven't shown up either. It's been 6 weeks now and I'm on leave. Anyone else dealing with this kind of delay? Any advice on how to speed things up?

I found a way to actually get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video explaining it: https://youtu.be/1X-mEsLtbmQ
