Finally got EDD to explain missing doctor forms for my disability claim - crazy process!
After 6 weeks of waiting for my SDI claim to process, I was going INSANE trying to figure out what was wrong. I kept getting vague messages about incomplete medical certification, but my doctor swore they submitted everything correctly through SDI Online!\n\nIn desperation, I drove almost 90 minutes to what I THOUGHT was an EDD office in Oakland (spoiler: it wasn't) but ended up being some other government building. But here's the crazy part - the receptionist was incredibly helpful and actually got an EDD representative on the phone with me right there!\n\nTurns out there's a completely separate medical form my doctor needed to fill out that they never mentioned in any of my online notifications. The EDD rep said they mailed it weeks ago but it never arrived. Now they have to mail me ANOTHER form that I need to physically take to my doctor, have them complete it, and then mail back to EDD.\n\nI've been without income this whole time because of some random paper form that nobody told me about! Has anyone else dealt with this mystery "different medical form" situation? The rep called it something specific but I was so frustrated I didn't write it down.

omg SAME THING happened to me last yr!! i think its the supplemental medical form they need when theres some kinda question about ur disability? my doc office was so confused bc they already sent in all the regular paperwork. took me like 3 extra weeks to get paid bc of this stupid paper form!

Did they ever explain WHY they needed this extra form? My doctor is already annoyed about filling out more paperwork and I'm worried this will delay everything even longer. Did they backpay you for all the weeks once it was finally approved?

This is EXACTLY why the EDD system is completely broken! They deliberately make the process confusing and use paper forms from the stone age to delay your benefits. I had the same issue last year and ended up having to apply for emergency assistance because my benefits were delayed THREE MONTHS due to some stupid form that nobody told me about until I threatened to contact my state representative. The whole system is designed to deny legitimate claims and save the state money. Did they even bother to call or email you about this missing form??? Of course not!!!

yep its a joke. my neighbor works 4 the state (not edd) & even she says its all about reducing their claim numbers. they make it so frustrating that ppl just give up claiming benefits they deserve

LOL I know right?? It was actually some county administrative building in Oakland. I was SO embarrassed when I walked in and realized it wasn't EDD at all, but the receptionist said people make that mistake all the time. Google Maps totally misled me! But she was amazing and actually had some direct line to EDD that got through after only about 15 min wait.

One thing to know about the supplemental form - it has to be filled out by the SAME doctor who completed your original certification. I made the mistake of having my specialist fill it out when my primary care doctor did the first form, and EDD rejected it completely. Had to start over with the right doctor and lost another 3 weeks of processing time.\n\nAlso make sure your doctor includes their medical license number and the correct diagnosis codes (ICD-10). When u finally get your claim approved, make sure to check if they calculated your benefit amount correctly. Mine was based on wrong quarters which gave me $340 less per week than I should have gotten. Had to file a whole separate appeal just to fix the calculation!! EDD is a mess right now with all the new claim system updates in 2025. Everything takes twice as long as it should.

This is an important point. Your SDI benefit amount is based on the highest-earning quarter in your base period (typically 5-18 months before your claim). You can request a recalculation if you believe they used incorrect wage information by submitting a DE 1545 form along with proof of your earnings (paystubs, W-2s, etc.).

since ur going thru the paper form nightmare ask the edd person on the phone if they can put a note in ur file that everythings coming. doesnt always work but sometimes helps speed things up when the form finally gets there!! also i made copies of EVERYTHING b4 mailing just in case they \

I went through exactly this last month! After getting the form and having my doctor fill it out, I brought it directly to an actual EDD office instead of mailing it. They accepted it and entered it into their system right there. My claim was approved 3 days later! Not all offices will do this, but it's worth calling ahead to check if they will. Saved me weeks of waiting for mail processing.

I went to the Sacramento office, but I called first to confirm they'd accept it. Not all offices handle disability stuff in person - some are unemployment only. Definitely call before making the drive! The number on their website actually worked for me when I selected the
