EDD SDI payment delays since May: Missing form, 'qualified' status, no deposit - Help!

I'm having some serious issues with my disability benefits and I'm at my wits end. I haven't been paid since May and now I'm dealing with a bunch of problems. First, I was supposed to get a form for continued disability benefits, but it never showed up. The deadline was last Saturday and now my next payment is gonna be delayed. On top of that, I finally got a payment processed but it's not showing up in my account. And to make things even more confusing, the payment status says 'qualified' instead of 'payment'. Has anyone else dealt with this mess? Any advice on how to get this sorted out?
