Can I print EDD SDI extension form online for doctor to sign instead of visiting office?

Hey everyone, my disability leave is almost up but I'm definitely not ready to go back to work yet. My doctor agreed to extend my disability period today, but she needs the official EDD form to complete. I remember having to go to the EDD office in person last time to get paperwork (what a nightmare that was), but with gas prices being what they are, I really don't want to drive all the way to the San Diego office if I don't have to. Can I just print the extension form from the EDD website and bring that to my doctor to sign? Will EDD accept a printed version or does it need to be the original they mail out? Has anyone done this successfully? Thanks in advance for any help!!

Yes u can print it!! I did this twice for my back injury. Just go to the EDD website, look for the section with SDI forms, and download the 'Claim Extension Form' (I think its DE 2525XX or something). Print it out, take it to ur doc, then either mail it back or u can upload it through ur SDI Online account which is WAY faster. Saved me so much hassle not having to drive to the actual office. The printed version is EXACTLY the same as what they would give u in person.

Just to provide some clarification, the form you're looking for is the "Physician/Practitioner's Supplementary Certificate" (DE 2525XX). You can absolutely print it yourself from the EDD website. The doctor needs to complete it fully, including their license number and contact information. Make sure they sign and date it. After your doctor completes it, you should submit it immediately through your SDI Online account by uploading it as a document. This is much faster than mailing it. The printed version is identical to what you'd receive in person, and EDD accepts these routinely. I recommend keeping a copy for your records in case of any processing issues.

Printing it from the website should work fine. just make sure your doctor signs it properly with the correct date! My extension got delayed by THREE WEEKS because my doctor put the wrong date (he put the date he filled it out instead of the date my disability was supposed to be extended from). So frustrating!!!

You know what's REALLY fun? When you print the form, give it to your doctor, they sign it, you upload it, and then EDD claims they never received it!!! Happened to me TWICE during my pregnancy disability claim. Then they make YOU responsible for getting another form filled out even though THEY lost it. The whole system is a joke.

When i had my disability extension for my anxiety/depression, my therapist just wrote a letter explaining why I needed more time off. EDD rejected it and told me I HAD to use their specific form. So definitely make sure you're getting the right form from the website!!! Don't make the same mistake I did lol

wait i'm confused why r u getting an extension from ur doctor? don't they just certify u for disability when ur actually at the appointment? or r u talking about state disability not private? i've never had to get a separate extension form but maybe i'm doing it wrong???

Thanks everyone for all the great advice! I found the DE 2525XX form on the website and printed it out. My doctor filled it out today and I just uploaded it to my SDI Online account. The site gave me a confirmation number so hopefully that means it went through correctly. I also took a screenshot of the confirmation page just in case (thanks for that tip!). Fingers crossed the extension gets processed quickly because my benefits are set to run out next week. I'll update if I run into any issues!

I had a similar situation where my benefits were about to expire and even though I got the confirmation number, it took them almost 2 weeks to actually process my extension. Luckily I called after a week and they were able to expedite it. Also, if you do have trouble getting through on the phone, try calling around 12:30-1:00pm too - sometimes there's a lull during lunch hour when it's easier to get connected.
