Letter from ESD asking questions - can I respond online instead of mailing back?

Hey everyone, I just got this letter from ESD asking me to fill out some questionnaire about my services/work. The letter says if I answer these questions online I don't need to mail or fax it back. Problem is, I can't figure out WHERE on the ESD website to do this! I've logged into my eServices account and clicked around but don't see anything about pending questionnaires or forms to complete. Has anyone dealt with this recently? I really don't want to mess up my claim by not responding, but I also don't want to mail it if I can do it electronically like they mentioned.

those letters r confusing af! i think u need to look under 'tasks' or 'alerts' when u login. sometimes they hide that stuff

Thanks, I'll check there again. The weird thing is I don't see any alerts or tasks showing up at all when I log in. My dashboard just shows my claim status but nothing about pending forms.

I had something similar happen last month. What you're probably looking at is an employment services questionnaire. You need to go to your eServices account, click on the 'UI Claim' option, then scroll down to where it says 'Pending Issues'. If you see your questionnaire listed there, you can click on it to fill it out online.\n\nIf you don't see anything there, check if there's a specific identification number on the letter - sometimes you need to enter that somewhere to access the right form.

This is helpful! I just checked and there's nothing under Pending Issues either. The letter has a reference number at the top right corner - maybe I need to enter that somewhere? The frustrating part is the letter specifically says I can do this online but doesn't say WHERE online!

Those questionnaires usually appear under your active alerts in your eServices account. Sometimes they take 1-2 business days after you receive the physical letter to show up in the system. If you don't see it by tomorrow, I'd recommend calling ESD directly to ask where exactly to find it online.\n\nMake sure you're also checking the correct claim if you have multiple claims. I've seen people miss questionnaires because they were looking at their most recent claim instead of an older one that needed information.

Exactly this! The online system is SO BAD about showing where to find things. I waited 4 days once and it never showed up online. Had to mail the stupid form anyway. What a waste of time!!!

The type of letter matters too. Is this a fact-finding questionnaire? A job search review? An adjudication question? Each one might appear in a different section of your eServices account.\n\nIf it's about services you provided (like if you're a contractor or had self-employment), it's likely an employment services questionnaire that should appear under 'Pending Issues' or sometimes under a section called 'Decisions and Determinations' which then has a subsection for outstanding questions.\n\nIf you've looked everywhere and still can't find it, I recommend using Claimyr to connect with an ESD agent quickly - I used them last month when I had a similar issue (https://claimyr.com). They have a video showing how it works: https://youtu.be/7DieNd3C7zQ?si=26TzE_zGms-DODN3. They got me connected to an actual person at ESD who told me exactly where to find my form.

Thanks for the detailed help! It's definitely about services/work I provided. I'll check the 'Decisions and Determinations' section as well. If it's still not there tomorrow, I might try that Claimyr service you mentioned - waiting on hold with ESD for hours isn't something I have time for right now.

Listen I've been threw this EXACT THING in January!!! What happened was they sent me letter but online form wasn't available for THREE DAYS after I got the mail!! ESD is totally disorganized with there systems. Just keep checking and if you don't see it after 3-4 days THEN mail it in. Don't miss the deadline whatever you do because they will mess up your claim!!!

Ugh, that's good to know about the delay! The letter says I have 10 days to respond, so I've got a little time. I'll keep checking online daily.

I went through the same thing when I was on unemployment last year. So frustrating! One thing no one mentioned - sometimes you get the letter but the online form is already EXPIRED. It's ridiculous but happens. When you're in eServices, go to

This is a really good point. I've also seen questionnaires appear under completely different categories than expected. The ESD website navigation is not intuitive at all.

Just checking back - were you able to find the online form? If the form is about services provided while on unemployment, it should definitely be in your eServices account somewhere. Sometimes they label these as \

Thanks for checking back! Good news - the form finally appeared in my account this morning under 'Additional Information Requested' like someone suggested. I've filled it out and submitted it online. The whole process took like 5 minutes once I actually found it. What a relief!
