PA UC asking for wage verification form after approval - why am I being questioned about my separation?
So I'm pretty confused right now. My unemployment claim was approved 2 weeks ago (got the determination letter and everything), but now they just sent me this wage verification form out of the blue. The weird part is it's asking questions about why I no longer work at my previous job - stuff I ALREADY answered during the initial application process. Why are they asking me about separation reasons AFTER already approving my claim? Has this happened to anyone else? Is this normal procedure or should I be worried they're trying to reverse my approval? I've already received one payment so I'm really confused and anxious about this.

This is actually pretty normal in PA's system. They do post-approval verification checks on a percentage of claims. It's nothing to worry about as long as the information you provide matches what you initially reported. Think of it as a random audit - they're just double-checking the facts. Make sure you fill it out completely and submit it by the deadline to avoid any issues with future payments.

•Thank you, that makes me feel better. I was worried they found something wrong with my claim! Do you know if they need any supporting documentation with the form? I have copies of my termination letter if that would help.

OMG same thing happened to me last month! Got approved, got 2 payments, then BAM - form asking why I left my job. I FREAKED OUT thinking they were gonna make me pay everything back. I called like 50 times trying to get through to ask why they were questioning me AFTER approving me. Complete waste of time - never got through!!

•Did you end up filling out the form? Did it affect your payments at all?

Yeah I filled it out - just repeated exactly what I said on my initial application. Payments kept coming no problem. I think its just some stupid bureaucratic thing they do to catch people who lie. As long as ur consistent it should be fine.

•This is why I hate dealing with unemployment! They make EVERYTHING so complicated. They could just check this stuff BEFORE approving claims but nooooo that would make too much sense!! 🙄

If you received a form that asks about separation after approval, it's likely a Form UC-45 Wage Verification. This is standard procedure and part of their quality control measures. PA UC randomly selects claims for additional verification even after initial approval. My advice: 1. Fill it out exactly as you did your initial application 2. Submit it by the deadline (usually 7 days) 3. Include any supporting documentation if you have it (termination letter would be helpful) 4. Continue filing your weekly claims during this process As long as your answers are consistent, your benefits should continue uninterrupted.

•Yes, that's exactly the form! UC-45. Thanks for the detailed explanation. This makes me feel much better - I was afraid I did something wrong. I'll make sure my answers match exactly what I said before.

Hey I just went through this exact situation last month! After trying for DAYS to get through to UC to ask about this form, I finally found a service called Claimyr that got me through to an agent in about 25 minutes instead of calling endlessly. The agent confirmed it's just a random verification thing they do - nothing to worry about. Check out their site at claimyr.com if you need to talk to someone at UC directly. Their video explains how it works: https://youtu.be/CEPETxZdo9E?si=WL1ZzVZWG3KiHrg2

•Is this legit? Sounds too good to be true... the PA UC phone system is a nightmare

•100% legit - saved me hours of redial hell. Agent told me these verification forms go out to about 10% of approved claims, totally random.

I think PA's system is so screwed up right now they don't even know what they're doing anymore lol. My neighbor got approved then denied then approved again all within 3 weeks!!! MAKE IT MAKE SENSE.

•That sounds awful! Did your neighbor have to appeal or did they fix it automatically?

One thing to note when filling out this verification form - be very precise about your last day worked. If you put a different date than what was on your initial application (even if off by just a day), it can trigger an investigation. Also, use exactly the same wording for your separation reason as you did originally. Consistency is key here.

•That's really good advice - I'll double-check my original application to make sure the dates and reason match exactly.

my freind said they just started doing these checks more after all the fraud during covid. its annoying but i guess makes sense

•This is correct. PA UC implemented enhanced verification procedures after the pandemic-related fraud issues. While it creates extra steps for legitimate claimants, it's actually helping ensure the system remains funded for those who truly need it.

UPDATE: I filled out the form yesterday and just checked my portal - everything still shows active and my next payment is scheduled as normal. Thanks everyone for the reassurance! Seems like this really is just a random verification thing.

Glad to hear it! Just remember to keep doing your weekly work search activities and certifications while this verification is processing. Two work search activities per week is still required, and missing those could cause more issues than the verification form itself.

•Definitely staying on top of the work search! I've been keeping detailed records of every application and follow-up.

I went through this exact same thing about 6 months ago and it scared me to death! Got my approval letter, started receiving payments, then suddenly got that UC-45 form asking about my separation again. I called myself crazy thinking I must have missed something or filled something out wrong the first time. Turns out it really is just their random quality control process - like others said, about 10% of approved claims get selected. I filled it out with exactly the same info as my original application and never had any issues. The key thing is just being consistent with your answers and getting it back to them quickly. Don't stress too much about it - sounds like you're handling it perfectly!

•Thank you so much for sharing your experience! It's really reassuring to hear from someone who went through the exact same thing. I was definitely overthinking it and worried I had made some mistake on my original application. Reading everyone's responses here has been such a relief - it sounds like this is way more common than I realized. I'm feeling much more confident about the whole process now!

I'm dealing with something similar right now! Got my approval about 3 weeks ago and just received what looks like the same UC-45 form yesterday. I was panicking thinking they were going to take back my approval, but after reading all these responses I feel so much better. It's crazy how they don't explain this process anywhere - like a simple note saying "this is routine verification" would save everyone so much stress! Going to fill mine out today with the exact same info I provided initially. Thanks to everyone who shared their experiences here, it really helps to know this is normal!
